  • It is important that you talk about your wishes so that others know what is important to you. The palliAGED advanced care planning page will help you to understand more.
  • It can be an emotional and stressful time when someone is nearing the end of their life. The Australian Government website Carer Gateway provide resources for planning for the end of life to help support you.
  • Advance care planning Australia provide information and forms to document your preferences and to prepare should you no longer be able to communicate your decisions.
  • Share your Advance care plan with others by using My Health Record. Visit the website and scroll down to find instructions on how to log in, control who can access your record and manage your own information.
  • Making a will ensures that your wishes are expressed regarding your estate. Wills services are available at the Public Trustee. Managing your finances is explained in the Living with Illness section of palliAGED.
  • It is likely that you may have a social media account for catching up with friends. Thinking about how you will manage your social media accounts after your death may be important to you and your family. Plan your digital legacy with these useful tips from the Australian Government eSafety Commissioner.

Lesbian, gay, bisexual, transgender and intersex

  • Planning your end-of-life care can feel like a challenge. The Southern Cross University has developed a booklet Respect my decisions: IT’S MY RIGHT! (383kb pdf) Designed specifically for the LGBTI community to plan for end of life.
  • The MOSAIC app is designed to empower lesbian, gay, bisexual, transgender and intersex people to better plan and manage their own care. Its aim is to support you with active decision making and provide resources for LGBTI people, their partners and families of choice.
