Supporting Services
Demand for aged care is growing both in residential aged care and the aged care packages for those living in their home. With an ageing population, demand will continue to grow and the need for care at the end of life will also grow. Aged care services will be actively involved in supporting increasing numbers of older people not only as they age but as they decline and die. End of life has been recognised in the strengthened aged care standards with a specific outcome focused on palliative care and end of life care (Outcome 5.7) and in the Support at Home Program which includes an end-of-life pathway.
Services need to consider how they can train and support their multidisciplinary workforce, establish care processes that can identify and respond to changing needs, and facilitate compassionate conversations. palliAGED resources can help services to be prepared to support their residents, clients, families and their staff.
