Needs Assessment

A needs assessment supports personalised palliative care by identifying physical, emotional, social, and spiritual needs. Regular reassessments, especially after health changes or hospital stays, keep care up to date. Using validated tools helps you recognise needs early and guide referrals. By monitoring needs as part of your daily practice, you can improve care planning, support families, and enhance end-of-life care.
